In most Western house systems it sets the cusp of the 10th house, and the midheaven sign camped there describes your public trajectory, career, reputation, the shape of ambition, not who you are in private.

The Astronomy: Where the Ecliptic Meets the Meridian

The local meridian is the great circle running from the north point on your horizon, straight up through the zenith, and down to the south point. Once a day, every degree of the ecliptic crosses it. The MC is whichever degree is crossing on the upper, visible half of that circle at your birth moment, the same reason the Sun is highest in the sky at local noon.

Is the Midheaven Always the 10th House Cusp?

No. In quadrant systems, Placidus and Koch, the two most common in Western practice, the MC is by definition the exact 10th house cusp. In whole-sign houses, the 10th house is simply the tenth sign counted from the ascendant, and the MC degree can fall inside a different house, occasionally even a different sign, than the one it nominally anchors.

Vedic (Jyotish) astrology works almost entirely in whole-sign houses. The karma sthana, its 10th house of action, career, and public standing, is counted from the ascendant sign, not from the MC's exact degree. The two systems can diverge by several degrees, sometimes by a full sign, on the same birth data. Neither reading is the correct one: they measure career through slightly different geometry, and a full chart, house by house, considers both.
